The Massachusetts MOLST Form

Signing the MOLST form is one possible outcome when a seriously ill patient and his or her clinician and loved ones engage in a process of advance care planning.

The MOLST form contains valid medical orders signed by a clinician and patient, that are based on the patient’s treatment preferences. 

Here are some important things to keep in mind about the MOLST form:

  • The MOLST form may instruct either to attempt or not attempt treatment.
  • Patients should be reassured that they will be made as comfortable as possible near the end of life, whether they choose to receive treatment or not.
  • The MOLST form is based on the patient's current health and treatment wishes.
  • The MOLST form goes into effect immediately after it is signed. 
  • Before establishing MOLST, patients should understand their medical condition and discuss their values and goals for care with their clinician and loved ones.
  • Before establishing MOLST, clinicians should explain all treatment options to the patient and his/her loved ones, including the treatment's use, benefits/burdens and possible outcomes.
  • Patients can cancel or ask to generate a new MOLST form at any time.
