Communicating with and caring for patients who are nearing the end of life requires special knowledge and skills that are often not a part of clinical training. 

In particular, facilitating appropriate advance care planning conversations with patients and their loved ones — the kinds of conversations that must precede the completion of a MOLST form — can be daunting.
