Communicating with and caring for patients who are nearing the end of life requires special knowledge and skills that are often not a part of clinical training.

In particular, facilitating appropriate advance care planning conversations with patients and their loved ones — the kinds of conversations that must precede the completion of a MOLST form — can be daunting.

The following resources are available to help health care providers learn more about palliative and end-of-life care — or refresh their current practice as it relates to patient care:

The End of Life/Palliative Education Resource Center

ELNEC - End-of-Life Nursing Education Consortium

American Academy of Hospice and Palliative Medicine
