MOLST information for Health Professionals

In 2010, MOLST was piloted in demonstration sites in the Greater Worcester area. In 2011, the use of MOLST began to expand to other areas of Massachusetts.
Use of MOLST is supported by the Massachusetts Boards of Registration in Medicine, Nursing and Physician Assistants as an appropriate way to meet the standard of care for communicating patient preferences about life-sustaining treatment options across care settings.
SAMPLE MOLST Form
The MOLST form is signed based on goals for care discussions between a clinician and patient or patient's representative. The MOLST form contains medical orders based on a patient's current medical condition and treatment preferences.
